Cancellation Policy

Informed Consent and Waiver I desire to voluntarily engage in a yoga program at Empowered Yoga and Coaching, LLC. I understand all the risks involved in participating in this yoga program. I understand it is my responsibility to consult a physician regarding my participation in all yoga programs offered by Empowered Yoga and Coaching prior to attending my first class. I understand that I am responsible for monitoring my own condition throughout yoga classes. If any unusual symptoms should occur, I will cease participation in this activity until medical advice has been acquired. I understand that I am participating in programs that may involve hands-on adjustments to my body. I am permitting all teachers and assistants from Empower Yoga and Coaching, LLC to touch and adjust my body. In signing this consent form, I affirm that I have read this form in its entirety and that I understand the nature of a yoga program and that to the best of my knowledge, I have presented all relevant health information to the instructor and listed it on this form. I agree to assume all the risk of this yoga program, and further release and agree to indemnify and hold harmless Empower Yoga and Coaching, LLC and the instructors conducting the yoga classes of any and all claims, suits, losses or related cases of action for damages, including but not limited to injury or death, accidental or otherwise during or arising in any way from the yoga program.
